The Boston Athletic Association (BAA) partnered with medical professionals across the Commonwealth, the Hopkinton Health and Public Safety Departments and the Massachusetts Department of Public Health (MDPH) to develop a layered COVID-19 policy that was protective of the participants, race volunteers, residents, and spectators. All Boston Marathon participants are required to provide proof of vaccination or produce a negative COVID-19 test in order to participate in the fall race. While masks will not be required while running, they will be enforced on transportation and in other areas to align with local guidelines. The majority of the volunteers are fully vaccinated. Symptomatic volunteers have been advised to remain at home. As the event activities are out of doors, CDC and MDPH publications support that the risk of race related COVID-19 transmission is extremely low.

In September 2020 the BAA announced a covid-19 Medical & Event Operations Advisory Group. Read more info here
